CARE advances self-care among and care delivery for older adults in our communities by:
- Developing an innovative transition to practice program for nurses entering long-term care settings, especially in rural communities
- Providing tools to help older adults and their caregivers prepare for and navigate a range of common late-life transitions
- Offering educational opportunities to front-line providers with limited professional development options, such as certified nurse assistants and personal care workers
- Supporting healthy aging in place for older adults with intellectual disability, by informing caregivers, long-term care staff and health care professionals about the capabilities of and challenges facing this group
